Struggling with Prolapse or Leaks? A Pessary Could Be A Game-Changer!
Do you feel a heavy sensation “down there”?
It might be all of the time or it might get worse towards the end of the day. You might also feel it more with certain activities such as lifting heavy weights or running– or even just carrying the baby all day.
Or maybe you’re tired of leaking every time you cough or sneeze? You might be ok with an occasional sneeze but during allergy season – you have to wear a panty liner daily!
I know these are common complaints from women. Something that is tossed around as almost a “right of passage” once you’re “of a certain age” or after you’ve had “so many kids”.
Some studies suggest that at least 1 in 4 women deal with some sort of prolapse or incontinence (this might even be a little skewed because not everyone admits it!). But actually, it isn’t something that women just have to deal with.
A simple, nonsurgical tool called a pessary could be a game-changer and might be exactly what you need!
What Is a Pessary?
A pessary is a device you can insert into your vagina, similar to how you can insert a cup or disc when on your period. A pessary helps to support the pelvic organs which decreases stress on the ligaments and pelvic floor muscles.
There are different types of pessaries available including a ring, dish and a cube. An evaluation is necessary to determine which one would best suit your needs.
Who Might Benefit From a Pessary?
Patients that have the following diagnoses and/or symptoms may benefit from a pessary:
Pelvic Organ Prolapse
Symptoms can include:
Heaviness
Pelvic pressure
Feeling of a bulging sensation
Feeling like something is falling out
Stress Urinary Incontinence
Leaking urine with:
Sneezing
Coughing
Laughing
Running
Jumping
Other physical exercises
Postpartum or Perimenopausal Women
Some women may benefit from a pessary during these times of transition due to:
Weakened pelvic floor muscles
Hormonal changes
Return to exercise support or prevention of worsening symptoms
Those Avoiding Surgery or Wanting a Conservative Option
May not be a candidate for surgery due to medical history
Grade of prolapse may not be appropriate for surgery despite symptoms
Inconsistent symptoms –May have more symptoms around your period or only when working out
Want symptom relief while working on pelvic floor strength
Signs You Might Need A Pessary
You’ve been told you have prolapse
You avoid lifting (during workouts or during daily life)
You can’t stop thinking about or feeling the “heavy” or “falling out” feeling
Running feels like something you’ll never be able to do again
You have to wear pads daily or often for leaking
What to Expect From a Pessary Fitting
A pessary fitting can be done by a trained pelvic floor therapist. During a pessary fitting, measurements will be taken to assess the type and size of pessary that would work best for your symptoms. Although these measurements are taken with care, sometimes it can take trialing a few sizes or types to ensure a good fit.
After a pessary is inserted, you should not be able to feel it. If you have any pain or discomfort, it is likely not a good fit. You should also be able to urinate, jump, lift and move with it inserted – it shouldn’t shift, move or fall out. Your therapist should also make sure you can remove it as needed and recommended.
Pessary + Pelvic Floor Therapy = Better Together
A pessary could be a game-change—but it’s not a cure. While it can offer immediate support and symptom relief, it doesn’t address the root cause of prolapse or leaking. That’s where pelvic floor therapy comes in.
Working with a pelvic floor therapist while using a pessary can help you:
Strengthen the muscles that support your pelvic organs
Improve how your body manages pressure (especially during movement or lifting)
Identify and adjust habits, posture, and movement patterns that may be contributing to your symptoms
With a pessary providing support, many people feel empowered to return to higher-level exercise—which further boosts strength, function, and confidence.
What would you love to get back to doing without symptoms?
Prolapse and incontinence can feel frustrating and even overwhelming—but you do have options! A pessary and pelvic floor therapy is a conservative and effective approach to managing symptoms, improving strength, and restoring confidence in your body.
